Program Manager

Responsibilities

• Scheduling, supervising and training all staff
• Attending Individual Service Plan (ISP) meetings and running all staff meetings
• Assisting with medical appointments
• Serving as liaison to families, care providers and all other support staff for individuals living in the home
• Assisting in the development of individual habilitation plans and ensuring treatment plans are complete
• Providing assistance to staff and supervisor(s) with individuals' activities of daily living as needed
• Ensuring pertinent information is communicated to appropriate staff (clinicians, quality assurance, care coordination, etc)
• Ensuring staff complete required documentation

Requirements

• High school diploma or GED
• Two (2) years' experience in ID/DD field
• One (1) year of supervisory experience
• Valid NYS driver’s license which meets agency guidelines

$19.70-$26.61

Schedule

Full time, 40 hours/week (on-call rotation required)
