Responsibilities
- Scheduling, supervising and training all staff
- Attending Individual Service Plan (ISP) meetings and running all staff meetings
- Assisting with medical appointments
- Serving as liaison to families, care providers and all other support staff for individuals living in the home
- Assisting in the development of individual habilitation plans and ensuring treatment plans are complete
- Providing assistance to staff and supervisor(s) with individuals' activities of daily living as needed
- Ensuring pertinent information is communicated to appropriate staff (clinicians, quality assurance, care coordination, etc)
- Ensuring staff complete required documentation
Requirements
- High school diploma or GED
- Two (2) years' experience in ID/DD field
- One (1) year of supervisory experience
- Valid NYS driver’s license which meets agency guidelines
