Key Responsibilities
- Oversee daily operations across all residential sites, ensuring services align with CSSPs and SPAs
- Maintain real-time awareness of program status, staffing, and resident needs
- Lead admissions, discharges, and program transitions
- Develop reports, track program metrics, and contribute to strategic planning
- Partner with HR and leadership on staffing needs and scheduling solutions
Designated Manager (245D) Compliance
- Ensure full compliance with MN Statute 245D, including service agreements, SPAs, and required timelines
- Oversee assessments, service planning, documentation, and annual reviews
- Maintain audit readiness and respond to licensing reviews, investigations, and corrective actions
- Ensure proper incident reporting and adherence to the Vulnerable Adult Act
- Train and support staff on regulatory requirements and service delivery standards
Care Coordination & Case Oversight
- Coordinate services across providers, agencies, and treatment teams
- Lead support team meetings and ensure timely follow-up on medical and psychiatric care
- Monitor individual outcomes and adjust services based on progress and needs
- Advocate for person-centered supports and appropriate funding/services
Quality Assurance & Performance Improvement
- Conduct audits, monitor documentation, and ensure compliance across programs
- Identify trends, risks, and improvement opportunities
- Lead quality improvement initiatives and corrective action plans
- Ensure consistent, high-quality service delivery and documentation practices
Staff Leadership & Development
- Supervise and develop program staff, including coaching, performance management, and accountability
- Partner with HR on training, discipline, and staff development
- Provide ongoing training in mental health, de-escalation, and person-centered care
- Model professionalism, consistency, and strong leadership across all programs
Collaboration & Communication
- Serve as liaison with families, case managers, healthcare providers, and regulatory agencies
- Build and maintain strong external partnerships
- Communicate clearly across all stakeholders and advocate for persons served
Qualifications
- Bachelor’s degree in Human Services, Healthcare Administration, or Social Work - or - RN
- 2+ years of supervisory experience in behavioral health or human services
- Experience supporting individuals with SPMI and co-occurring conditions
- Strong knowledge of MN 245D regulations and adult foster care licensing
- Demonstrated leadership, organizational, and problem-solving skills
Additional Requirements
- DHS background study clearance
- Valid driver’s license
- Ability to participate in on-call support as needed
Preferred Experience
- Familiarity with residential documentation systems (e.g., Residex)
Starting Pay
$73,000 - $85,000 per year plus benefits - dependent on licensure, education and 245D experience
