Core Accountabilities
- Caseload Management
- Assure strategies set by family and team are carried out
- Document results for progress towards goals
- Assure people can manage finances, seek financial assistance, maintain possessions, arrange transportation, and be active in community
- Assure people live their best lives by providing assistance with grooming, hygiene, personal care, and medical needs
- Provide direction and support to staff to help people achieve optimum health
- Ensure staff monitor and document health information
- Aid staff in administering medications and scheduling/appending appointments
- Encourage balanced healthy meals and diets
- Conduct quarterly visits and monthly contacts with people supported
- Document meetings, conversations, and important information
Work Environment & Physical Requirements
- Sit for extended periods of time
- Walk and stand
- Use hands and fingers to handle, feel, or operate standard office equipment
- Reach with hands and arms
- Lift up to 50 pounds
- Communicate in person, by telephone, and virtually
- Use vision abilities including close and distant focus
- Be exposed to noise levels ranging from quiet to moderate, and occasionally loud sounds and distracting noise levels
- Regularly drive throughout assigned region, with occasional statewide travel
Required Qualifications
- Bachelor’s Degree in human service field or related
- Preference given to psychology, social work, education, or gerontology degrees
- One year of experience in working directly with people with developmental disabilities
- Become certified QDDP (Qualified Developmental Disability Professional)
- Become certified in CPR and First Aid or Therapeutic Responses
Pay and Schedule
$50,000 per year, Full-time, Exempt
