Essential Job Functions
- Schedules medical appointments and follow-ups
- Makes sure medications are reviewed for accuracy, ordered, and discarded appropriately
- Coordinates and follows up with professionals like PT, OT, SLP, and Nutritionist
- Ensures medical forms are completed before leaving the physician’s office
- Follows up on medical recommendations and ensures the Program Coordinator is aware of them
- Ensures all medical forms are completed in full before leaving the physician’s office
- Follows up on phone contacts with medical providers and other professionals
- Ensures food, supplies, and equipment are properly stored and maintained
- Maintains cleanliness and sanitation of the home and grounds
- Maintains Evergreen vehicles, including cleaning, fuel, and routine maintenance
- Makes sure individuals served are free of abuse, neglect, and mistreatment
- Provides input to the Program Coordinator regarding direct care staff performance
- Works unscheduled shifts as needed or assists the Program Coordinator in finding suitable staff
- Makes sure consumer petty cash, budget requirements, and expenditures are within limits
- Monitors individual spending and reviews written reports
- Makes sure policies, rules, and regulations are followed and reports any violations
- Makes sure transportation requirements are met and resolves issues in the absence of specific instructions
- Aids the Program Coordinator in maintaining and documenting specific training for individuals served
Qualifications/Experience/Job Knowledge
- A combination of education or experience equivalent to a two (2) year degree in one of the behavioral sciences or experience in supporting individuals with developmental/intellectual disabilities
- Prior supervisory experience preferred
- Working knowledge of Windows 7, Office 2010 applications
- Aptitude to learn other software programs as required for this position
Physical Requirements
- Constantly moves about to coordinate work
- Regularly works in a fast-paced environment with multiple task deadlines
- Occasionally exposed to viruses and infectious conditions
- Constantly alert and observant during working hours
Special Requirements
- Must have a good driving record meeting the minimum requirements for Evergreen and reliable transportation
- Must pass a drug screen and criminal background check
- May be required to attend seminar or job-related training courses
- Must have understanding, patience, and tact in dealing with individuals with intellectual and developmental disabilities
- Must have the ability to maintain good working relationships with Evergreen staff and with contacts from other agencies or entities
- Must be able to prioritize work tasks
