About the role
Lifeworks is a nonprofit human service agency dedicated to providing adults with developmental disabilities the opportunity to live engaged and meaningful lives. As a Program Coordinator, you will play a crucial role in coordinating, developing, and implementing rehabilitation services for individuals served by Lifeworks Employment Services.
Responsibilities
- Promotes optimal health for individuals through routine, preventive, and emergency health care and clinical services.
- Safeguards work environments ensuring they are safe, secure, and free from harm.
- Supervises and performs case management responsibilities, including preparing, implementing, and evaluating Individual Support Plans for employees funded by various sources.
- Collaborates with the DDS Case Management Team Service Coordinators to ensure coordinated rehabilitation and production efforts.
- Serves as the program Safety Officer, implementing safety plans and programs, and encouraging regular communication and cooperation among staff.
- Maintains effective communication with residential providers and families regarding activities and supported employment placements.
- Coordinates the scheduling of daily and weekly staff and employee work assignments, for all individual, enclave supported employment placements, and Life Enhancement activities.
- Coordinates the weekly transportation schedule for all employment sites.
- Coordinates the evaluation and intake of referrals with the leadership team.
- Provides coverage support as necessary for various sites, placements, activities, and outings in the community.
- Provides training, support, and follow-up to individual and group placements.
- Maintains accurate records for individuals served for the purpose of program evaluation, billing, etc.
- Carefully coordinates accurate payroll records for all individuals and employees, job coaches, and relief staff.
- Aids the leadership team in long-term and short-term planning, organization, and evaluation of services and in the daily operation of the program.
- Implements Quality Enhancement standards within the program.
- Coordinates the scheduling of all relief staff for the Program.
- Performs other duties and/or assignments as needed for work/program locations based on the needs of the company.
Requirements
- Experience working with Individuals with ID/DD
- Bachelor's degree from four-year college or university and one to two years related experience
- Passes Fingerprint background check, CORI, DPPC, and Driving background checks
- Valid Driver's License required
- Excellent verbal and written communication skills
