About the role

Transitions Healthcare Autumn Grove is hiring PRN Therapists (Physical Therapist, Occupational Therapist, Physical Therapy Assistant, or Occupational Therapy Assistant) to supplement full-time hours or provide a flexible schedule. Responsibilities

• Screen, evaluate, and treat residents needing services.
• Implement treatment plans through direct treatment and education of treatment staff.
• Evaluate results of therapy.
• Complete discharge planning.

Requirements

• Licensed as an Occupational Therapist (OT), Physical Therapist (PT), Physical Therapy Assistant (PTA), or Occupational Therapy Assistant (OTA) in the Commonwealth of Pennsylvania in good standing.
