PRN RN Case Manager

About the role

You’ll join a cross-functional team that sits at the heart of Unifi Healthcare’s mission: delivering accessible, compassionate, and patient-centered care in the home. This role is part of a growing network of clinicians, technologists, and operations specialists who are reimagining how healthcare is delivered across the U.S. You’ll work closely with caregivers, patients, and partners to reduce complexity, streamline workflows, and ensure every individual receives the support they need to live longer, safer lives at home.

Responsibilities

• Design, implement, and refine processes that improve patient outcomes and caregiver experience.
• Collaborate with clinical, technical, and operational teams to integrate new services and scale existing ones.
• Analyze data to identify gaps in care, inefficiencies, or opportunities for innovation.
• Develop and maintain partnerships with external providers, payers, and community organizations.
• Advocate for patient needs and ensure solutions align with Unifi’s mission of personalized, home-based care.

Qualifications

• Experience in healthcare operations, clinical care, or a related field (e.g., home health, care coordination, telehealth).
• Strong problem-solving skills and the ability to work in a fast-paced, evolving environment.
• Empathy and a deep commitment to patient-centered care.
• Excellent communication and collaboration skills, with the ability to bridge gaps between technical and non-technical teams.
• Passion for improving healthcare accessibility and reducing complexity for patients and caregivers.
