PRN RN Case Manager
About the role
RN Case Managers work collaboratively with inter-professional teams across the continuum of care to facilitate and ensure effective transitional care coordination.
Responsibilities
- Ensure compliance with standards of the accrediting bodies, state mandated revised code and the CMS Conditions of Participation.
- Promote interprofessional synchronization throughout the acute episode.
- Complete effective handoffs to the next level of care or community to ensure continuity of care.
- Proactively assess, identify and work to mitigate the risk posed by barriers to patients’ transitions of care.
- Work with the patient and/or care partner(s) to develop a comprehensive, individualized transitional care plan.
- Participate and lead in patient rounding activities, huddles and committee meetings.
Requirements
- Minimum qualifications: Graduate from an accredited school of professional nursing (Diploma, ADN, BSN program), Current state licensure as a Registered Nurse (RN), Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross, 3 years of recent RN experience.
- Preferred qualifications: Bachelor of Science in Nursing (BSN), Certification by an approved entity within one year of eligibility (CTM, ACM, CCM or AHA), 2 years of recent experience in utilization review, quality or care management, Direct acute case management experience (if not applicable, orientation 5 days per week is required).
Qualifications
- Requires full range of motion, manual and finger dexterity and eye-hand coordination.
- Requires standing and walking for extensive periods of time.
- Requires normal or corrected hearing and vision to normal range.
- Sedentary work exerting up to 10 pounds of force occasionally and /or a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects, including the human body.

Pay
The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set and education. The pay range displayed does not include any applicable pay practices (e.g., shift differentials, overtime, etc.). The pay range does not include the value of Cleveland Clinic’s benefits package (e.g., healthcare, dental and vision benefits, retirement savings account contributions, etc.).
Schedule
A caregiver in this position will work between 8:00am and 4:30pm averaging about 40 hours plus a weekend shift in 6 weeks or 3 weekend shifts in 6 weeks. Holidays requirements are twice per year.
Orientation
Department orientation occurs Monday through Friday, 5 days per week for 6 weeks.
