PRN RN Case Manager

Role Overview: This position provides PRN care management support, focusing on advocating for patients' healthcare needs, promoting responsible health practices, and ensuring patient safety. Key Responsibilities: Develops individualized care plans by identifying barriers and setting specific goals.Manages complex medical cases through thorough assessment and resource deployment.Facilitates safe transitions across care settings, including home, hospital, and post-acute facilities.Utilizes telehealth, virtual visits, and field-based support to deliver care.Collaborates effectively with interdisciplinary teams and engages patients and families to align care with their values and goals.Addresses gaps in care, leverages community resources, and ensures confidentiality and ethical standards.Documents activities accurately and works independently within scope of license. Qualifications & Skills: Registered Nurse with current licensure and a Bachelor’s degree or higher in Nursing.Preferred CCM or RN-BC certification (attainable within 36 months).Strong communication, interpersonal, and technology skills, including proficiency in Microsoft Office.Knowledge of managed care, quality improvement, and pathophysiology.Ability to work in dynamic environments, perform assessments, and prioritize tasks effectively. Work Environment: Fast-paced, adaptable, primarily telephonic or virtual visits, with occasional clinic or hospital interactions.
