The RN Case Manager ensures quality patient care through development, implementation, and evaluation of individual patient care plans within the home setting. The Case Manager serves as the liaison between the Agency and patient families, managing and coordinating clinical activities while adhering to professional standards of care and Agency Policy & Procedures.

Responsibilities

  • Performs documentation and chart review to ensure accuracy and adherence to the client’s Plan of Care (POC).
  • Submits clinical documentation (weekly billing and payroll information) within specified deadlines.
  • Maintains accurate and updated information in QuickBase for all assigned clients.
  • Manages care implementation with a focus on quality patient care and family satisfaction.
  • Coordinates care authorization with Authorization Specialist(s) for new admissions and re-certifications.
  • Implements, evaluates, and oversees individual patient POCs aligned with physician’s orders through assessments and consultations with health team members.
  • Provides education to clients regarding benefits, utilizing internal and external resources (e.g., DME, home modifications, medication prior authorizations).
  • Maintains ongoing responsibility for an assigned caseload.
  • Attends pertinent medical appointments with clients (or coordinates coverage) as needed.
  • Builds rapport with patient families to effectively manage care.
  • Assists the staffing department with scheduling workloads for efficiency and meets deadlines.
  • Maintains open communication with DON, ADON(s), other Case Managers, RN Authorization Specialists, non-licensed staff, and ancillary services.
  • Notifies Scheduling Department and ADON(s) of field-related issues involving clients or employees.
  • Communicates critical information (e.g., deaths, hospitalizations, changes in condition) via email to relevant teams.
  • Provides clinical guidance and coaching to staff and patient families, holding field staff accountable for care quality.
  • Attends monthly Case Management meetings and participates in staff in-service programs/continuing education.
  • Participates in company-designated committees and client home visits as required.
  • Abides by and promotes all Agency Policies & Procedures.
  • Supports and ensures compliance with state and federal program regulations through process development and evaluation.
  • Maintains professional nursing competencies and adheres to accepted standards/scope of practice.
  • Participates in Case Management teams to ensure continuity of care.
  • Delivers exceptional customer service and compassion to clients and families with professionalism.
  • Ensures availability during company office hours for accessibility to patients, families, staff, and colleagues.
  • Participates in performance reviews and goal-setting for personal and company growth.
  • Advocates for and coordinates DME supplies and necessary equipment/services.
  • Fills in as the RN in emergent situations when all other avenues are exhausted.
  • Follows Chain of Command and acceptable protocols in all situations.

Requirements

  • Current RN licensure.
  • Minimum 3 years of progressively responsible clinical experience.
  • Excellent writing, organizational, and time management skills.
  • Prior management or leadership experience.
  • Ability to work independently, multi-task, prioritize, and function in a team environment.
  • Professional, courteous, compassionate, and customer-oriented demeanor.
  • Discretion, trustworthiness, and high level of integrity.
  • Excellent computer and internet skills.
  • Ability to perform bending and lifting required for patient care.
  • Ability to safely lift 40-50 lbs.
  • Professional attire (scrubs and nametag preferred) when visiting clients.
