You will play a critical role in promoting independence, ensuring safety, and enhancing quality of life for our clients and their families. Key Responsibilities Direct Patient Care Provide compassionate personal care including bathing (bed bath, shower, or tub), dressing, grooming, oral care, and hygiene supportAssist with mobility including ambulation, wheelchair use, and safe transfersSupport toileting needs, including catheter and ostomy careReposition bedridden clients and maintain clean, comfortable beddingAssist with meal preparation and support nutritional intake as needed Clinical Responsibilities Monitor and record vital signs (heart rate, blood pressure, respiratory rate, temperature)Document intake and outputAdminister and manage medications as prescribedAssess client nursing needs on an ongoing basisProvide nursing services in accordance with the established care planPromptly report changes in condition, concerns, or progress to the Case Manager and physicianComplete accurate and timely clinical documentation Care Plan Collaboration Participate in the development and implementation of individualized service plansObserve and report changes in mood, behavior, or emotional statusCommunicate effectively with interdisciplinary team members Supportive Services Perform light housekeeping to maintain a safe, healthy environment (laundry, vacuuming, dusting, kitchen tasks)Organize client living spaces to promote safety and comfortAssist with errands and transportation, including medication pickup and grocery shopping Professional Standards Practice strictly within legal scope of licensureMaintain professional appearance and boundariesPromote teamwork, unity, and a culture of respectAdapt to additional responsibilities as assigned by leadership Working Environment Daily travel to client homes and occasional visits to the agency officeFlexible scheduling, including potential weekends and holidays (with compensation provided)Must be able to lift at least 25 poundsWork environment varies based on individual client needs Minimum Qualifications Current, valid RN or LPN/LVN licenseReliable personal vehicle with valid insuranceClear criminal background check and negative pre-employment drug screeningStrong verbal and written English communication skills Why Join Us?
