About the role
This role involves creating personalized treatment plans for residents in a Skilled Nursing Facility setting.Responsibilities
- Restore independence by developing and implementing individualized treatment goals.
- Educate and support patients, families, and caregivers on treatment plans and safe mobility techniques.
- Evaluate and document each resident's progress accurately and in a timely manner.
- Serve as a key communicator between the clinical team and the patient's support network.
Requirements
- Active Physical Therapist (PT) license in the state.
- Previous experience in a Skilled Nursing Facility is preferred.
- Strong verbal and written communication skills to advocate for your patients.
