What You'll Do
- Restore Independence: Develop and implement individualized treatment goals to improve strength, mobility, balance, and daily living activities.
- Educate & Support: Train patients, families, and caregivers on treatment plans, safe mobility techniques, and progress.
- Evaluate & Document: Conduct thorough assessments and maintain accurate, timely documentation of each resident's progress.
- Collaborate Effectively: Serve as a key communicator between the clinical team and the patient's support network.
What You'll Need
- Active Physical Therapist (PT) license in the state.
- Previous experience in a Skilled Nursing Facility is preferred.
- Strong verbal and written communication skills to advocate for your patients.
