PRN Physical Therapist (PT)

About the role

As an Enhance Rehabilitation Physical Therapist, you will be a part of a dynamic team providing the highest quality care for patients in a skilled nursing facility (SNF) setting.

Responsibilities

• Evaluate and treat patients following an individualized plan of care
• Provide excellent care for our patients
• Provide excellent customer service to patients and family members
• Collaborate with facility staff to ensure excellent care for patients
• Evaluate new patients, develop plans of care and set measurable, functional goals
• Treat patients, answer their concerns and educate them on their injury, progress and probable outcomes
• Supervise Physical Therapist Assistants
• Evaluate patient progress, revise plans of care and reset goals
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes

Requirements

• Masters degree in Physical Therapy from an accredited program
• Current license to practice physical therapy in the state of [State]
• At least 1 year of clinical experience in a SNF setting preferred
• Excellent communication skills
• Ability to work independently and as part of a team

Qualifications

• Passing score on the National Physical Therapy Examination (NPTE)
• Knowledge of rehabilitation techniques and protocols
• Strong interpersonal skills
• Proficient in computer software for documentation and record-keeping
