Job Summary

The Physical Therapist PT will provide exceptional, personalized care in our skilled nursing facility, addressing each resident's unique needs through treatment, education, and training.

Responsibilities

• Motivate patients during treatment
• Auxiliary residents to function optimally
• Promote a healthy lifestyle by improving strength, flexibility, balance, and coordination
• Establish and carry out a comprehensive treatment plan
• Provide education and training to patients, families, and caregivers regarding diagnoses, progress, and long-term functional outcomes

Requirements

• An active state Physical Therapist PT license
• Some SNF experience
