Responsibilities
- Affords assistance to patients, significant others, physicians, and healthcare team members to comprehend significant personal, emotional, environmental, and social factors and issues impacting the patient's health conditions.
- Contributes to the formation of comprehensive, integrated treatment plans for patients.
- Instructs healthcare team members about community resources available to support patients.
- Monitors and reports changes in the client's emotional and social factors affecting their health and the need for care and their reaction to treatment.
- Maintains and submits written clinical records as necessary, including initial evaluations, care plans, and daily notes.
- Evaluates clients and their families' responses to and effectiveness of medical social work interventions.
- Participates in agency activities and committees, such as staff development activities and in-service education.
- Serves as a supervisor for the social work assistant once monthly.
- Fosters personal and professional growth through continuous learning, including formal education, workshops, conferences, and professional meetings.
Skills Required
- Able to assess the client's capacity to handle their social and health challenges.
- Functions as a consultant to healthcare team members, assisting them in understanding the social, emotional, and environmental factors linked to the client's health issues.
- Supports clients in utilizing the resources within their families and communities.
- Provides rehabilitative and supportive casework.
- Aids clients and their families in managing personal and environmental difficulties that might lead to illness or hinder access to optimal medical care.
Job Conditions
- The ability to communicate effectively, both verbally and in writing, is essential.
- The capability to access clients' homes, which may not be routinely wheelchair accessible, is required.
- Hearing, vision, and physical dexterity must be sufficient for performing a physical assessment of the client's condition and providing client care.
- Physical activities may include walking, sitting, stooping, standing, and minimal to maximum lifting of clients and turning of clients.
Qualifications Required
- A person holding a master's degree (MSW) from an accredited school of social work by the Council on Social Work Education, or being licensed in the State of Texas.
- One year of experience working as a social worker.
- Current driver's license with valid automobile insurance.
