PRN/Part Time - Medical Social Worker (MSW) - Home Health - Mason, TX
Responsibilities
Assists the patients, significant others, physician, and health care team staff to understand significant personal, emotional, environmental, and social factors and difficulties related to the patient's health problems.
Contributes as a health care team member to the development of comprehensive, integrated treatment plans for patients.
Instructs health care team members on community resources available to assist patients.
Observes records and reports changes in the client's emotional and social factors that affect the client's illness and his/her need for care and his/her response to treatment.
Maintains and submits written clinical records as deemed by the Agency, including the initial evaluation, the care plan and daily notes.
Evaluates the clients and family's response to, and effectiveness of, the medical social work intervention.
Participates in Agency activities and committees when appropriate to include staff development activities and in-service education.
Supervises the social work assistant once monthly.
Requirements
A person who has a master's degree (MSW) from a school of social work accredited by the Council on Social Work Education, or the individual must be licensed in the State of Texas.
One year of experience, working as a social worker.
Current driver's license with current automobile insurance.
Qualifications
The ability to communicate well, both verbally and in writing is required.
The ability to access clients' homes which may not be routinely wheelchair accessible is required.
Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
Physical activities will include, walking, sitting, stooping, standing and minimal to maximum lifting of clients and the turning of clients.
