Responsibilities
- Affords assistance to patients, significant others, physicians, and healthcare team members to comprehend significant personal, emotional, environmental, and social factors and difficulties related to the patient's health issues; these factors can impede the full utilization of medical services and the care plan.
- Contributes to the formation of comprehensive, integrated treatment plans for patients, collaborating with the healthcare team.
- Instructs healthcare team members about community resources available to support patients.
- Monitors and documents changes in the client's emotional and social factors that influence their health condition and their need for care, as well as their reaction to treatment.
- Maintains and submits clinical records as necessary, including initial evaluations, care plans, and daily notes.
- Evaluates clients and their families' responses to and effectiveness of the medical social work intervention.
- Participates in agency activities and committees, such as staff development and in-service education.
- Serves as a supervisor for the social work assistant once monthly.
- Continuously strives to enhance their medical social work practice through formal education, attending workshops and conferences, participating in professional and related organizational meetings, and conducting individual research and reading.
Skills Required
- Affords assessments of the client's capacity to cope with their social and health problems.
- Functions as a consultant to the healthcare team, assisting them in understanding the social, emotional, and environmental factors associated with the client's health issues.
- Assists clients and their families in utilizing the resources of their families and the community.
- Provides rehabilitative and supportive casework.
- Affords assistance to clients and their families in managing personal and environmental difficulties that might predispose them to illness or hinder their access to optimal medical care.
Job Conditions
- The capability to communicate effectively, both verbally and in writing, is essential.
- The ability to access clients' homes, which may not be routinely wheelchair accessible, is essential.
- Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to provide client care.
- Physical activities will include walking, sitting, stooping, standing, and minimal to maximum lifting of clients and the turning of clients.
Qualifications Required
- A person holding a master's degree (MSW) from an accredited school of social work by the Council on Social Work Education, or an individual licensed in the State of Texas.
- One year of experience working as a social worker.
- A valid driver's license with current automobile insurance.
