About the role
Responsible for the evaluation, clinical assessment, treatment plan, implementation, and progression of goal-directed, purposeful Occupational Therapy interventions. Responsibilities also include consultations with medical staff and health professionals, completion of medical records and insurance forms, and supervision of Rehab associates.
Responsibilities
- Prescribes and instructs patients in therapeutic self-care and creative activities that promote functional independence with attention to both physical and mental components.
- Performs evaluation procedures as per Policy and Procedure: self-care assessment, FIM assessment, ROM, manual muscle test, upper extremity/hand assignment, sensory testing, interest checklist, splint/orth assessment, position assessment, and vital signs.
- Educates patient and/or responsible party to home programs and the use of assistive devices.
- Affirms learning and provides reinforcement or implements further teaching strategies as indicated per policy and procedure.
- Provides care, based on physical, psychosocial, communication, safety, and education level and related criteria for the infant, pediatric, adolescent, adult, geriatric patient per the established age-specific standard.
- Evaluates home environment for concerns, plan of treatment.
- Complies with all Home Health/ standards including but not limited to visits and communication.
- Completes plan of treatment and recertification as per Medicare guidelines.
- Completes SOC, Recertification, and Discharge per guidelines.
- Performs CPR when directed by the patient’s/appropriate representative’s wishes.
- Displays safe, efficient, and prudent use and maintenance of D.M.E. and medical supplies and adaptive equipment.
- Familiar with supply requisition systems as per policy and procedure.
- Performs treatments/procedures according to specific standards.
- Performs and monitors patient teaching according to the policy and procedures.
- Captures care coordination with the care team in order to facilitate a change in the level of care in compliance with the change in level of care standard.
- Identifies the stages of death and dying.
Qualifications
- Current Occupational Therapist Licensure as required by the state where services will be performed: MD: Occupational Therapy licensure through the Maryland Department of Health & Mental Hygiene OH: Occupational Therapy licensure through the Ohio Occupational Therapy, and Athletic Trainers Board PA: Occupational Therapy licensure through the State Board of Occupational Therapy Education and Licensure WV: Occupational Therapy licensure through the West Virginia Board of Occupational Therapy
- Valid driver’s license.
- CPR certification within 30 days of hire date.
