About the role

Be part of a uniquely specialized team of caregivers providing hospice and palliative care in the patient's home or care facility. This is a PRN (as-needed) position with one of the leading providers of post-acute care.

Responsibilities

• Deliver compassionate, patient-centered hospice and palliative care services.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Provide symptom management, emotional support, and end-of-life care to patients and their families.
• Conduct assessments, monitor patient progress, and adjust care plans as needed.
• Educate patients, families, and caregivers on hospice and palliative care practices.
