PRN Palliative Care Nurse Practitioner

Position: PRN Nurse Practitioner providing palliative care in Jackson and surrounding counties. Key Responsibilities Evaluate patient eligibility for palliative care services.Assess the needs of patients and their families to ensure comprehensive care.Coordinate with physicians and interdisciplinary teams to develop and update care plans.Conduct regular reassessments of patients to monitor condition and adjust treatment accordingly.Perform home visits to deliver personalized care in patients' residences. Qualifications & Skills Current Nurse Practitioner licensure with relevant clinical experience.Strong assessment and care planning skills.Excellent communication and collaboration abilities for interdisciplinary teamwork.Ability to provide compassionate, patient-centered care in home settings.
