Responsibilities

• Evaluate patient eligibility and assess patient and family needs.
• Coordinate care with physicians and interdisciplinary teams.

• Conduct home visits to deliver care.

Qualifications & Skills

• Licensed Nurse Practitioner with experience in palliative or hospice care preferred.
• Strong assessment and communication skills.
• Ability to collaborate effectively with healthcare teams.
• Comfortable performing home visits and managing patient care independently.
