About the Role

A Medical Social Worker (MSW) providing intermittent social work services in patients' homes under physician orders and supervision.

Responsibilities

• Develop and coordinate social services for patients.
• Act as a resource to the healthcare team.
• Maintain confidentiality and participate in care planning.
• Prepare and maintain documentation.
• Engage with community resources to support patient needs.
• Ensure adherence to organizational policies and promote quality initiatives.
• Support patients' emotional and social needs.
• Participate in ongoing education and team conferences.

Requirements

• Master's or doctoral degree in social work from an accredited school, or a baccalaureate degree with healthcare social work experience.
• One year of home care experience preferred.
• Strong communication skills, initiative, independence, and ability to collaborate effectively.
