About the Role

PRN Home Health Physical Therapy Assistant (PTA) dedicated to providing personalized care to restore patient mobility and independence in a home setting.

Responsibilities

• Delivering therapeutic exercises and functional training
• Applying physical therapy techniques and modalities to reduce pain and restore function
• Monitoring and documenting patient progress for safe, effective care
• Teaching patients and families to use assistive devices and follow home exercise programs
• Collaborating with PTs, nurses, aides, and the care team to enhance patient outcomes

Qualifications

• Associate's degree from an accredited PTA program
• State exam completion and licensure
• Valid driver’s license with insured, well-maintained vehicle
