Responsibilities

• Evaluates occupational therapy patients to determine rehabilitation needs, potential for achievement, and develops a plan of care.
• Participates in the implementation and development of the Plan of Care to ensure quality, continuity of care, and proper discharge planning.
• Makes the initial therapy evaluation visit and re-evaluates patients' therapy needs during each visit, following OASIS and PPS regulatory guidelines.
• Communicates significant findings, problems, or changes in condition to supervisors, physicians, and other team members.
• Initiates plans for patient safety using patient, family, and community resources.
• Verifies the Plan of Care prior to each visit and provides care according to physician’s orders, assessment data, and established standards.
• Writes physician orders for additional visits and changes to the Plan of Care, per agency policy.
• Incorporates patient care goals into therapy care as documented in therapy notes.
• Performs occupational therapy services in accordance with accepted standards of practice and physician certification.
• Instructs patients and families in "activities of daily living" using adaptive equipment, sensory integration, and fine motor coordination skills.
• Assists patients and caregivers in securing necessary equipment, including fabrication of custom-made splints.
• Instructs patients and caregivers in the therapy home program, providing modifications and clarifications as needed.
• Counsels and instructs patients and families in meeting therapy and related needs.
• Supervises and provides clinical direction to Certified Occupational Therapy Assistants (COTA), Home Health Aides (HHA), and clinical staff.

This role operates in a physically demanding, high-stress environment with a full range of body motion, including handling, lifting, and transferring patients. You may work irregular hours, including on-call shifts.

Requirements

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted OT license valid for the state of work.
• Minimum one (1) year of experience in a clinical care setting.
• Able and willing to travel within the branch/office coverage area.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel up to 50% of the time.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
