Home Health Occupational Therapist PRN

Position Overview Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions in home health settings. Responsibilities include conducting initial and ongoing assessments of mobility, strength, and function, interpreting tests, developing and updating personalized care plans, and ensuring compliance with physician orders. The OT will document progress, modify goals, and coordinate services with healthcare professionals, patients, and caregivers. Supervision of auxiliary personnel and completion of OASIS assessments are also required.
