About the Role
As a Home Health Occupational Therapist, you will work one-on-one with patients in their homes to help them regain independence in daily activities. You will assess functional abilities, design personalized treatment plans, and develop adaptive strategies to support safety and independence. Collaboration with patients, families, and care teams is key, providing education, hands-on training, and encouragement to promote confidence and self-sufficiency.
Responsibilities
- Conduct patient assessments and develop individualized treatment plans
- Implement therapeutic interventions to improve daily living skills
- Educate patients and families on safety, adaptive equipment, and routines
- Collaborate with interdisciplinary teams to ensure comprehensive care
- Travel within service areas for home visits and care delivery
Requirements
- Graduate of an accredited occupational therapy program (ACOTE)
- Current state license as an Occupational Therapist
- Minimum of 1 year of relevant experience
- Strong clinical assessment, problem-solving, and communication skills
- Ability to work independently and adapt to varied home environments
- Reliable transportation and willingness to travel daily
Preferred Experience & Knowledge
- Home health experience
- Experience with adaptive equipment and home modifications
- Knowledge of Medicare documentation and home health regulations
Pay
Pay ranges from $84,000 to $89,000 USD.
