About the role
We are seeking a Home Health Occupational Therapist to provide personalized rehabilitation care in patients' homes.
Responsibilities
- Collaborate with physicians and the care team to develop and adjust therapy plans focused on daily living, mobility, strength, and coordination.
- Participate in care coordination and discharge planning.
- Train staff and families on managing therapy routines.
- Submit evaluation reports and treatment plans.
- Supervise therapy assistants and aides to ensure quality care and proper documentation.
Qualifications
- Current, unrestricted North Carolina Occupational Therapy license.
- Valid driver’s license, auto insurance, and CPR certification.
- Participation in driver safety and TB screening programs.

- Home Health Settings.
