About the role

We are seeking a Home Health Occupational Therapist to deliver personalized rehabilitation care in patients' homes. The role emphasizes delivering compassionate, comprehensive care to help patients regain independence and improve their quality of life.

Responsibilities

• Collaborate with physicians and the care team to develop and adjust therapy plans.
• Focus on daily living skills, strength, coordination, and adaptive equipment needs.
• Participate in care coordination and discharge planning.
• Train staff and family members on managing therapy programs.
• Submit evaluation and progress reports.
• Provide instructions to therapy staff and oversee their performance.

Qualifications

• Unrestricted Occupational Therapy license in North Carolina.
• Valid driver’s license and auto insurance.
• CPR certification.
• Preferred experience in home health.

Requirements

• Adherence to safety and TB screening policies.
