Position Summary

The Occupational Therapist provides care to clients in their home according to the plan of care and with participation by the family, nurse, or other persons as necessary. All functions shall be performed in accordance with the established policies and practices of the agency.

Essential Duties

  • Guide and instruct the client in prescribing therapeutic self-care and create activities that improve independence and physical and cognitive functioning
  • Participate in the development of the plan of care
  • Prepare a diagnostic evaluation report of each client and make recommendations
  • Prepare clinical and progress notes
  • Advise, consult, and teach appropriate treatment procedures and supportive activities to clients, family members, and other agency staff
  • Establish household management routines
  • Design and/or order appropriate adaptive/assistive devices and/or splints to promote optimal level of functioning
  • Instruct clients and their families in activities of daily living
  • Attend care conferences as deemed necessary
  • Instruct the family in the client’s total occupational therapy program
  • Maintain a high degree of confidentiality at all times due to access to sensitive information
  • Maintain regular, predictable, consistent attendance and be flexible to meet the needs of the department
  • Follows all Medicare, Medicaid, and HIPAA regulations and requirements
  • Performs other duties as assigned

Position Requirements & Competencies

  • Graduate of an accredited occupational therapy curriculum
  • Certified as an occupational therapist by AOTA, American Occupational Therapy Association
  • Current Occupational Therapist license in the state in which practicing
  • Minimum one year of work experience as an Occupational Therapist
  • Valid Driver’s license
  • Current CPR and First Aid Certification
  • Demonstrate ability to exercise independent judgment with limited supervision
  • Comfortable working in a home health care setting within a client’s residence
  • Excellent verbal and written communication skills
  • Flexible schedule, including availability to work evenings, weekends, and holidays as needed
  • Well organized and excellent time-management and attention to details skills
