Key Responsibilities
- Conduct initial and ongoing assessments of patient needs
- Implement and adjust individualized care plans
- Ensure compliance with hospice policies and Medicare regulations
- Educate patients and families on self-care and end-of-life options
- Supervise paraprofessionals delivering patient care
- Document and report clinical findings and participate in interdisciplinary meetings
- Enhance patient satisfaction and experience
Qualifications
- Current Michigan LPN License in good standing
- Minimum of 1 year experience in hospice or palliative care
- Strong empathy and communication skills
- Flexibility to respond to varying patient needs
