About the role

The Licensed Practical Nurse (LPN) assists in planning, coordination, and care of hospice patients in accordance with the patient’s plan of care. The position provides direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the interdisciplinary team to help achieve positive care outcomes.

Responsibilities

  • Provide direct patient care as defined in State Nurse Practice Act following a patient assessment and an approved plan of care initiated by the registered nurse.
    • This position may be responsible for one or more of the following areas:
    • Focused/limited nursing visits
    • Telephonic triage
    • Night/weekend coverage back up rotation as shared among the nursing team in case the primary nurse is off
    • Other duties assigned by the Hospice Leadership
  • Collaborate with hospice team members regarding patient care.
  • Perform focused/limited nursing assessment, recognize signs and symptoms of end-stage medical conditions/illnesses and implements planned nursing and medical interventions as delegated by registered nurse/clinician.
  • Evaluate patient’s response to treatments/medications and notify hospice providers and/or registered nurses of concerns.
  • Contribute to the development, implementation, or modification of the individualized patient's plan of care under the direction of the registered nurse/clinician.
  • Provide health teaching/education and support to patients and caregivers related to end-of-life care needs.
  • Aid in hospice chart auditing, as requested.

Requirements

  • Graduate of an accredited LPN training program
  • Minimum one (1) year of experience as a licensed practical nurse in home health, hospice, skilled care facility, memory care unit, or acute care setting (preferably medical/surgical)
  • Current unencumbered LPN license in the State of Oregon
  • CPR certification at hire or within 6 months in position
  • Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle

  • Knowledge of Federal, State and Local regulations, Hospice Medicare Conditions of Participation, and HIPAA privacy rules and regulations
  • Knowledgeable about general computer skills and a working knowledge of Electronic Medical Records systems, MS office, Outlook, and the internet
  • Excellent interpersonal skills
  • Ability to tolerate and manage stress and grief
  • Ability to work both independently and collaboratively
  • Ability to work under pressure to meet deadlines
  • Ability to take initiative and utilize innovative techniques and ingenuity
  • Ability to interact with various departments throughout the organization
  • Ability to work flexible hours, if needed
  • Ability to plan, prioritize and coordinate multiple projects or tasks
  • Ability to adhere to organizational standards, policies and procedures
  • Ability to work effectively with diverse individuals and groups
  • Ability to learn, focus, understand, and evaluate information and determine appropriate actions
  • Ability to accept direction and feedback, as well as tolerate and manage stress
  • Ability to see, read, hear, and speak clearly for at least 6 hours/day
  • Ability to stand, walk, sit, and perform repetitive finger and wrist movement for at least 3-6 hours/day
  • Ability to lift, carry, push, pull, climb stairs, bend, and/or pinch small objects for at least 3 hours/day
  • Ability to operate a motorized vehicle in all kinds of weather conditions
