Private Duty RN for Assessments

About the role

Amaray Care is seeking a Registered Nurse to provide in-person assessments across the PG County area. This position requires travel and involves performing initial and ongoing home care assessments, developing and updating patient care plans, supervising caregivers, and coordinating patient services.

Responsibilities

• Initiate and update the plan of care based on doctor’s orders.
• Perform admission, transfer, re-certification, resumption of care, and discharge of home care patients.
• Observe, assess, and document patient symptoms and reactions.
• Educate patients and caregivers on disease processes, medications, plans of care, and individualized treatment plans.
• Coordinate patient services and supervise LPNs and CNAs.
• Notify physicians and other personnel of changes in the patient’s condition.
• Follow agency policy and procedures, as well as Medicare and Medicaid regulations.

Requirements

• Registered Nurse (R.N.) with a current license.
• Positive and favorable interaction with patients, families, and staff.
• Reliable transportation.

• Proficient in assessing patients’ conditions.
• Compliant with agency policies and procedures.
• Familiarity with Medicare and Medicaid regulations.

Skills

• Ability to perform initial and ongoing home care assessments.
