Responsibilities
- Deliver exceptional care to the dedicated client in their home
- Execute comprehensive personalized weekly schedules and care plans
- Provide skilled nursing services as prescripted under a client's plan of care
- Counsel clients and family in meeting nursing and related needs
- Meet all documentation requirements according to agency policy and procedure
- Coordinate services by participating in case conferences, case management, team meetings, and staff meetings as indicated
- Administer administrative functions such as documenting task completion and reporting changes in the client’s condition
Qualifications
- Registered Nurse (RN) license required
- BLS required
- Experience in placing peripheral IV's, accessing ports and working with PICC lines required
- Ability to use sound judgement and remain calm in a variety of situations
- Basic computer skills
- Proof of eligibility to work in the U.S.
