Responsibilities

• Deliver exceptional care to the dedicated client in their home.
• Help execute comprehensive personalized weekly schedules and care plans.
• Provide skilled nursing services as prescripted under a client's plan of care that can be safely provided in the home, within your scope of practice.
• Counsel clients and family in meeting nursing and related needs.
• Meet all documentation requirements according to agency policy and procedure.
• Coordinate services by participating in case conferences, case management, team meetings, and staff meetings as indicated.
• Administer medications and treatments as prescribed.
• Assess client's health status and document changes in condition.
• Ensure compliance with all safety protocols and regulations.

Qualifications

• Registered Nurse (RN) license required.
• BLS (Basic Life Support) required.
• Experience in placing peripheral IV's, accessing ports, and working with PICC lines required.
• Ability to use sound judgment and remain calm in a variety of situations.
• Basic computer skills.
• Proof of eligibility to work in the U.S.
