Responsibilities

• Deliver exceptional care to dedicated clients in their homes
• Help execute comprehensive, personalized weekly schedules and care plans
• Provide skilled nursing services as prescribed under a client's plan of care that can be safely provided in the home, within your scope of practice
• Counsel clients and family in meeting nursing and related needs
• Meet all documentation requirements according to agency policy and procedure
• Coordinate services by participating in case conferences, case management, team meetings, and staff meetings as indicated
• Complete administrative functions, including documentation of task completion and reporting changes in the client's physical condition, mental capability, or behavior

Requirements

• Registered Nurse (RN) license required
• BLS certification required
• Experience in placing peripheral IVs, accessing ports, and working with PICC lines required
• Ability to use sound judgment and remain calm in a variety of situations
• Basic computer skills
• Proof of eligibility to work in the U.S.
• Must comply with state background screening requirements
• Background screenings required
