Responsibilities

• Deliver personalized homecare to patients
• Document patient progress electronically
• Support patients' health needs in a variety of settings

Requirements

• Active, unencumbered LPN/LVN license in the practicing state
• Current CPR certification with hands-on component
• TB skin test within the last 12 months
• Minimum of 6 months hands-on clinical experience
• Reliable transportation
