Private Duty Nurse LVN - Weekend Days

About the Role

We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in patients' homes, under the supervision of a Registered Nurse. The role involves delivering and documenting patient care, assessing patient conditions, updating care plans, and implementing interventions to achieve positive outcomes. The nurse educates patients and families on disease management and self-care, ensuring compliance with safety and infection control standards.

Responsibilities

• Coordinate patient care, monitor and report changes in patient conditions
• Maintain accurate and thorough documentation
• Follow policies, procedures, and regulations
• Participate in ongoing education and skill development
• Maintain safety standards and provide patient education

Requirements

• Active, unencumbered LPN/LVN license in the practice state
• Current CPR certification
• TB test within the last 12 months
• Reliable transportation
• Relevant clinical experience preferred
