About the Role
We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings, under the supervision of a Registered Nurse. The role involves delivering quality patient care according to physician orders, assessing patient conditions, documenting activities, updating care plans, recognizing and responding to changes in patient needs, ensuring infection control, and educating patients and families on disease management and self-care techniques.
Responsibilities
- Coordinate patient care in home settings
- Monitor and report changes in patient conditions
- Document nursing actions and update care plans
- Implement physician-ordered care plans
- Ensure compliance with policies and regulations
- Maintain infection control standards
- Educate patients and families on disease management and self-care
Requirements
- Active, unrestricted LPN/LVN license
- Current CPR certification
- TB test within the last 12 months
- Reliable transportation
Prior hands-on nursing experience preferred but not mandatory.
