About the role
Licensed Practical / Vocational Nurse (LPN/LVN) responsible for providing compassionate, high-quality patient care in a home setting.
Responsibilities
- Deliver personalized nursing care to pediatric and adult patients.
- Utilize electronic documentation via tablets in patients' homes.
- Collaborate with a multidisciplinary team to ensure optimal clinical outcomes.
- Follow care plans and maintain compliance with health and safety standards.
Qualifications
- Active, unencumbered LPN/LVN license in the practicing state.
- Current CPR certification (training available post-hire if needed).
- TB skin test within the last 12 months.
- Reliable transportation.
- Preferred but not required: six months of hands-on nursing experience.
Key Duties
- Provide skilled nursing care in patients' homes.
- Maintain accurate documentation and adhere to care plans.
- Support patients’ health and safety using clinical expertise.
- Participate in ongoing training and skill development.
