About the role

Join a dynamic home healthcare team as a Licensed Practical / Vocational Nurse (LPN/LVN). Provide compassionate, patient-centered care in clients' homes, primarily during overnight shifts. Ensure clinical outcomes through hands-on nursing care and documentation via electronic systems. Collaborate with a multidisciplinary team to deliver quality homecare for children and adults. Participate in paid training, mentorship, and inclusive learning sessions to support professional growth.

Responsibilities

• Provide compassionate, patient-centered care in clients' homes, primarily during overnight shifts
• Ensure clinical outcomes through hands-on nursing care and documentation via electronic systems
• Collaborate with a multidisciplinary team to deliver quality homecare for children and adults
• Participate in paid training, mentorship, and inclusive learning sessions to support professional growth

Requirements

• Active, unencumbered LPN/LVN license in the practice state; compact licenses transferred within 90 days if applicable
• Current CPR certification with hands-on component
• TB skin test within the last 12 months
• Reliable transportation
• Preferred six months of hands-on nursing experience
