Private Duty LPN - Sevierville

About the role

As the Home and Community Based Services Licensed Practical Nurse, you will provide direct nursing care as prescribed by the physician to acutely ill and/or chronically ill patients in their homes. You will record clinical and progress notes, coordinate other patient care activities/services, and work under the direct supervision of a Registered Nurse or designated supervisor.

Responsibilities

• Provide high‑quality clinical services within the scope of practice, following state nursing laws and the patient’s plan of care.
• Coordinate care with other members of the health‑care team and follow all applicable infection‑control procedures.
• Document patient visits thoroughly in the electronic medical record, using the agency’s electronic visit verification program as required.
• Participate actively in the agency’s care‑coordination process, including timely reporting of patient needs and condition changes, attending case conferences, and communicating with patients, families, physicians, clinicians, agency staff, case coordinators, and community case managers.
• Function effectively in any home situation, regardless of the client’s age, race, creed, color, sex, disability, or financial condition.

Required Qualifications

• Current and unrestricted Licensed Practical Nurse (LPN) licensure in the state of practice.
• Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.

Preferred Qualifications

• Current CPR certification or ability to obtain it within 90 days of hire.
• Ability to work flexible hours as required to meet client needs.
• Ability to manage multiple tasks simultaneously and work independently.
• Strong communication, writing, and organizational skills.

Pay

The hourly pay for this role ranges from $22.90 to $34.35 per hour based on full‑time employment, local labor markets, education, experience, certifications, and other factors. The company complies with all applicable minimum wage laws.
