Responsibilities
- Providing quality patient care in home settings, ensuring outstanding clinical outcomes
- Using electronic documentation tools in patients' homes
- Collaborating with a team to deliver empathetic, personalized care
Qualifications
- Active, unencumbered LPN/LVN license in the practice state
- Current CPR certification (training available post-hire if needed)
- TB skin test within the last 12 months
- Reliable transportation required
- Six months of hands-on nursing experience preferred but not mandatory
