Responsibilities

- Utilize electronic documentation via tablets in patients' homes
- Collaborate with a multidisciplinary team

Qualifications
- Active, unencumbered LPN/LVN license in the practice state
- Current CPR certification (training available after hire)
- TB skin test within last 12 months
- Reliable transportation
