Responsibilities

• Provide high‑quality homecare for pediatric patients, including those with tracheostomy and ventilator support.
• Ensure outstanding clinical outcomes through compassionate and empathetic care.
• Utilize electronic documentation via patient tablets.
• Collaborate with a team of clinicians and support staff.
• Deliver personalized homecare, document care electronically, and support patients and families with professionalism.

Qualifications

• Active, unencumbered LPN/LVN license in the practice state.
• Compact licenses transferred within 90 days if applicable.
• Current CPR certification with hands‑on component.
• TB skin test within the last 12 months.
• Preferred: six months of hands‑on nursing experience.
• Reliable transportation required.
