Private Duty Home Health RN Needed

About the role

The Private Duty Registered Nurse (RN) at Haven Specialty Home Care provides skilled nursing services in patients' homes, supporting individualized treatment plans developed with physicians and patient/caregiver input. The RN manages a small patient caseload, working primarily independently to ensure patient safety, comfort, and effective care delivery while adhering to all regulatory and company standards.

Responsibilities

• Conduct initial and ongoing patient assessments using OASIS data elements.
• Develop, implement, and evaluate individualized care plans with physician and patient/caregiver collaboration.
• Provide skilled nursing interventions as prescribed in the care plan.
• Ensure patient emotional and physical comfort and safety, respecting cultural considerations.
• Document all observations, treatments, and progress accurately using Electronic Health Records (EHR).
• Maintain strict confidentiality and comply with HIPAA privacy regulations.
• Communicate physician orders and notify physicians of any unusual patient reactions or changes.
• Participate in case conferences, team meetings, staff meetings, and performance improvement initiatives as assigned.
• Uphold professional conduct and adhere to Haven Specialty Home Care policies and procedures.

Required Qualifications

• Minimum 1 year of nursing experience.
• Valid Nursing License in the state of North Carolina or a Multi-State License.

Preferred Qualifications

• Experience in home health care nursing.
• Proficiency in patient assessment and nursing documentation.
• Knowledge of HIPAA compliance and patient confidentiality.
• Strong communication, critical thinking, and teamwork skills.
• Ability to provide emotional support to patients and their families.

Schedule

8‑hour shifts, 8 a.m. – 4 p.m.

• 2 Full‑Time Registered Nurses (benefits offered to full‑time employees).
• 1 PRN/Part‑Time Registered Nurse.
