Private Duty LVN providing one-on-one skilled nursing care in patients' homes.

Responsibilities

• Deliver clinically focused care including assessments, medication administration, ventilator and tracheostomy management, and wound care.
• Closely monitor and document patient status.
• Collaborate with families and interdisciplinary team to follow individualized care plans.
• Educate caregivers and support patient comfort, safety, and independence.

Qualifications & Skills

• Licensed Vocational Nurse (LVN) license.
• Compassionate, patient-focused approach.
• Strong clinical skills and ability to manage complex care needs.
• Excellent communication and teamwork abilities.
