Role: Private Duty LVN providing personalized skilled nursing care in patients' homes. Responsibilities Deliver clinically focused care, including assessments, medication administration, ventilator and tracheostomy management, and wound care.Monitor, document, and report patient status accurately.Collaborate with families and interdisciplinary team to develop and follow individualized care plans.Educate caregivers to support patient safety, comfort, and independence. Qualifications & Skills Licensed Vocational Nurse (LVN) licensure.Compassionate, patient-focused approach.Strong clinical skills in pediatric and adult care settings.Ability to work independently and as part of a team.Excellent communication and documentation skills.
