Role: Private Duty LVN providing one-on-one skilled nursing care to pediatric and adult patients in their homes. Responsibilities Performing assessments, medication administration, and wound care.Managing ventilator and tracheostomy care.Monitoring and documenting patient status accurately.Collaborating with families and interdisciplinary teams to develop and follow individualized care plans.Educating caregivers to ensure patient safety, comfort, and independence.
