About the role
Provide one-on-one skilled nursing care in patients' homes, including assessments, medication administration, wound care, and monitoring chronic conditions.
Collaborate with families and care teams to follow detailed care plans, document accurately, and respond promptly to changes in patient conditions. Promote safety and independence, building meaningful, long-term relationships with patients in the community.
Requirements
- Licensed Practical Nurse (LPN) or Registered Nurse (RN)
- Compassionate, patient-centered approach
- Strong communication skills and attention to detail
- Ability to work independently
- Flexibility and adaptability to patient needs
